Communities of Practice

Communities of Practice or CoPs are groups of people who are committed to each other to offer support, share learning, and develop new knowledge in order to advance practice on a specific topic. This mechanism for knowledge exchange can be extremely effective when strong leadership, dedicated human resources (e.g. knowledge brokers, information specialists, resource coordinators, etc.), and engaged members all contribute to the perceived value and impact of a community of practice on knowledge transfer. See below current CoPs supported by CDRAKE.

CDRAKE has partnered with the Alzheimer Knowledge Exchange (AKE) to develop a Community of Practice (CoP) on the topic of Self-Management in Dementia. The AKE & CDRAKE Self-Management in Dementia CoP will address misconceptions and ensure effective use of self-management support strategies by health and social service providers and others who support people living with dementia by:

Primary Care is a cornerstone and often a gateway into the health system as it relates to older persons experiencing issues related to mental health and dementia. CDRAKE is working with the Alzheimer Knowledge Exchange in Ontario to offer primary care practitioners, interested in dementia care practices, with a forum for knowledge exchange to support their practice and learning needs.

The National Behavioural Support Systems (BSS) community of practice consists of approximately 30 leaders in seniors mental health and dementia representing almost all provinces and territories.  This community of practice has been engaging in a series of knowledge exchange opportunities since June, 2010.
