Learnings from the National Population Health Study of Neurological Conditions: Part II - Using models and databases to identify patients with neurological conditions and what needs and / or gaps exist for neurological populations across Canada.

Dr. Susan Jaglal, Vice-Chair Research and Professor in the Department of Physical Therapy at the University of Toronto and Dr. Karen Tu, Scientist at the Institute for Clinical Evaluative Sciences (ICES), an Associate Professor at the University of Toronto and a practicing family physician in the Toronto Western Hospital Family Health Team.

Dr. Jaglel, Vice-Chair Research and Professor in the Department of Physical Therapy at the University of Toronto will describe the needs and gaps in health and community based services for neurological populations across Canada and the model that was developed based on the themes identified via these needs and gaps.  The Chronic Care Model for Neurological Conditions (CCM-NC), builds on the Expanded Chronic Care Model and promotes intersectoral collaboration.
